Qualifying Exam Form (PDF file)

The Qualifying Exam Form should be given to your qualifying exam committee chairperson. This form serves as a record of completion of the qualifying exam.

It must be signed by every member of your qualifying exam committee. It is the chairperson’s responsibility to turn this form in to the Graduate Student Coordinator.

In addition to the signed form, the chairperson should submit a summary of the questions and responses from the examination.
