The Candidacy Presentation Form (PDF or Word) should be given to your doctoral thesis advisor. This form serves as a record of completion of the candidacy presentation. It must be signed by every member of your doctoral committee.

It is the thesis advisor's responsibility to turn this form in to the Graduate Student Coordinator.
