Used to request removal and disposal of hazardous materials. Waste removals performed on each Thursday of the week except for Holiday periods and removal request should be submitted by Monday prior to the removal need date. |

Used by the Principal Investigator to begin the process for Receiving authorization by the University's Radiation Safety Committee for the use and possession of radioactive materials. |
