| Who may participate? If you are a full time regular or fixed-term employee, or a part time regular or fixed-term employee scheduled to work at least 1,440 hours per year you are eligible to participate in the dental plan. What type of coverage is available? The dental insurance plan, administered by MetLife Dental, provides full coverage for the reasonable and customary cost of preventive services twice a year. |

| Other services are subject to an annual deductible of $35 per individual, $100 per family. Services, such as fillings, are paid at 80 percent of the reasonable and customary charge. Major services, such as bridges and crowns, are paid at 50 percent of the reasonable and customary charge. The maximum benefit per calendar year per individual is $1,500. There is a once-in-a-lifetime orthodontic benefit of 50 percent of covered dental charges up to a maximum benefit of $1,500 per person. You may choose individual, two-person, or family coverage. Dependent children up to age 19 (or to age 25, if full-time students) are eligible for coverage. When can I enroll?
