To obtain benefits, contact Human Resources at (518) 276-6302. Human Resources will provide information on employee's participation in benefit programs.

Dental Insurance: Coverage for a spouse and dependent children will continue until the end of the month during which the employee died. Information on continuation (available for up to 36 months at 102% of the group rate) will be sent to the spouse.
Action: To continue coverage, complete and return the election form.

Health Insurance: Coverage for a spouse and dependent children will continue until the end of the month during which the employee died. Information on continuation (available for up to 36 months at 102% of the group rate) will be sent. For those employees who were at least age 55 and who had five years of service, continuation of coverage is available for their spouses' lifetimes and until dependent children cease to be dependents. Rensselaer provides a credit for spousal coverage of 1.67% for each year of service up to a maximum credit of 50% and limited to $85 per month for spouses who are 65 or older.
Action: To continue coverage, complete and return the election form.

Life Insurance: Rensselaer provides eligible employees with 2.25 times the greater of an employee's July 1 salary or prior calendar year's compensation. Benefit includes an equal amount of accidental death and dismemberment insurance.
Action: Sign and return the form you will receive along with a copy of the employee's death certificate (with a raised seal). Benefit will be sent to you by the insurance carrier.

Long Term Disability Insurance: For employees who died while collecting a long term disability benefit, the beneficiary is entitled to a death benefit of three times the monthly disability benefit.
Action: None. Benefit will be sent to you by the insurance carrier.

Retirement: For Contributory Defined Benefit Retirement Plan: Beneficiary is entitled to five times the employee's accrued benefit.
Action: None

Retirement: For Defined Contribution Retirement Program: Distribution information is available from carries.
Action: Contact Fidelity Investments or TIAA-CREF.

Supplemental Retirement Program: Distribution information is available from carries.
Action: Contact Fidelity Investments, Scudder Funds, Principal Mutual or TIAA-CREF.

Tuition Benefits: If a spouse is enrolled in a course, the tuition benefit will continue until the end of the current semester. For a regular employee who has completed three years of full time continuous service, and whose child is currently receiving the Tuition Scholarship Benefit, that benefit will continue for the remainder of the eight undergraduate semesters. For a regular employee who has the equivalent of ten years of full time service and whose child has not yet enrolled in an undergraduate program, that child is eligible to receive the tuition benefit for eight undergraduate semesters.
Action: Submit a Tuition Scholarship Request Form to Human Resources.

Vacation: Regular and fixed term employees who were scheduled to work 20 or more hours per week and who were appointed for a period of at least 16 weeks earn vacation. Faculty and academic year coaches do not earn vacation. Any vacation pay to which the employee was entitled will be paid in accordance with the law. Per Rensselaer policy, fixed term employees are not paid for unused vacation.
Action: None.

Wages: Any wages to which the employee was entitled, but had not yet received will be paid in accordance with the law.
Action: None.
