Safety and Security Agreement

I, _____________________________________ understand that safety and security

(insert name)

in the Micro and Nano Fabrication Clean Room (MNCR) are the responsibility of everyone, including myself. I also understand that good safety and security practices include preventing unauthorized access to the MNCR, and preventing unauthorized use or theft of equipment and other property in the MNCR.

I agree that ...

I will not permit others to use my MNCR access card.
I will not allow anyone not having their own access card to enter the MNCR without permission from the MNCR Manager.
I will work safely and responsibly while in the MNCR, and I will abide by all aspects of the MNCR Safety Policy and other policies and procedures which have been explained to me.
I will only operate equipment for which I have been designated as a qualified operator.
I will not disable or otherwise defeat any equipment interlocks, modify any fume exhaust systems, or tamper with any alarm systems in the MNCR.

I understand that only authorized personnel may transport, change or adjust compressed gas cylinders in the MNCR.

I understand that only authorized personnel may transfer liquid nitrogen from the bulk storage unit to a dewar, or from a dewar to another container.

I also agree that ...

I will store clean room garments in a neat and orderly fashion.
I will complete log book entries when I begin and conclude usage of a tool.
In the event that I observe a potentially or actually hazardous situation such as a gas leak, a chemical spill or a fire, I will activate an alarm.
I will report all injuries, accidents and potentially unsafe situations to the MNCR staff.
I will assist with general MNCR operations when possible.

I have completed the MNCR safety training, and I have read and understood this agreement and the MNCR Safety Policy document.

I understand that anyone violating MNCR safety or security policies shall be subject to possible disciplinary action, including ...

Suspension or revocation of MNCR privileges (as determined by the Director of CIE).
Liability for cleanup and/or repair costs incurred by Rensselaer as a result of the safety violation.
Referral to the appropriate Dean or Department Chair for further disciplinary action. For a student, this may include referral to the Dean of Student and the Rensselaer Judicial System.

Advisors: By signing this agreement, I acknowledge that anyone who supervises the work of another and has prior knowledge of activities which violate MNCR or Institute safety policies shall also be considered to have violated the policies.
